Patient Forum Group

 

Over the last few years the Surgery has been developing a Patient Forum Group.

 

This is a small group of patients who meet twice a year to discuss the development and improvement of our services.

 

Our aim is to make sure our Patient Forum is representative of our patient population. To allow more people to share their views and get involved we recently set up a 'Virtual Patient Forum'.

 

Their points are then discussed at our Patient Forum by the patient representatives.

 

Our Patient Forum helped us choose a questionnaire to use in our recent Patient Survey, they have reviewed the results of the survery and based on your comments they agreed an action plan to help us improve our service. Please go to our Patient Forum section for more details.

SATURDAY MORNING SERVICE

CHACEWATER SURGERY

Did you know that we have a Saturday morning surgery

for people who work in the week?

These are pre-bookable appointments ONLY which need to be made in advance

You can also collect routine prescriptions on a Saturday

Any emergencies still need to be dealt with via the out of hours service which can be contacted by telephoning the Surgery

We are now able to send out text reminders to be sent to patients.

To ensure that this service works we do require patient’s mobile telephone numbers. If, however, you do not wish to receive reminders please let reception know and we can remove your details from the system.

The messages are sent the day before your appointment and will be sent during working hours. You will not be able to respond via text to this message. If you have any questions regarding this service please do not hesitate to contact us. There are forms available at reception to leave us your mobile numbers or decline to receive messages.

Please note - If we already have your mobile number on record you will need to notify us if you do not wish to receive these reminders.
