Welcome to Chacewater and Devoran Surgeries

The Surgeries are situated within the heart of their respective villages and our Practice Team of Doctors, Clinical Staff and Administration Staff feel privileged to be able to serve both communities in offering the highest standard of healthcare to all.

 
We offer regular clinics for a variety of services including Chronic Disease Reviews


•Hypertension, Diabetes, Heart, Asthma & Stroke Reviews 

•Well Woman and Well Man Health Check

•Minor Surgery including Cryotherapy

•Midwifery Service, 24hr and 8 week baby checks and Routine Childhood Immunisations

•Contraception Reviews including Coil Fittings, Implanon Contraceptive Device and Depo Injections

•Travel Vaccinations

In addition to this our Doctors offer annual health checks for patients with Dementia, Mental Health Problems, Learning Disabilities and also for patients who have more complex health concerns.

Your Summary Care Record

From Thursday 12th October Summary Care Records will be accessible unless you have previously opted out of this service.

Care professionals in England use an electronic record called the Summary Care Record (SCR). This can provide those involved in your care with faster secure access to key information from your GP record

What is a SCR? If you are registered with a GP practice in England, you will already have an SCR unless you have previously chosen not to have one. It includes the following basic information:
• Medicines you are taking
• Allergies you suffer from
• Any bad reactions to medicines.
It also includes your name, address, date of birth and unique NHS Number which helps to identify you correctly.

Who can see my SCR? Only authorised care professional staff in England who are involved in your direct care can have access to your SCR. Your SCR will not be used for any other purposes. These staff:
• Need to have a Smartcard with a chip and passcode
• Will only see the information they need to do their job
• Will have their details recorded every time they look at your record.


What choices do you have? You can now choose to include more information in your SCR, such as significant medical history (past and present), information about management of long term conditions, immunisations and patient preferences such as end of life care information, particular care needs and communication preferences.
If you would like to do this, talk to your GP practice as it can only be added with your permission.
Remember, you can change your mind about your SCR at any time. Talk to your GP practice if you want to discuss your option to add more information or decide you no longer want an SCR.

Care professionals will ask for your permission if they need to look at your SCR. If they cannot ask you because you are unconscious or otherwise unable to communicate, they may decide to look at your record because doing so is in your best interest. This access is recorded and checked to ensure that it is appropriate.

SCRs for children If you are the parent or guardian of a child under 16, and feel they are able to understand this information you should show it to them. You can then support them to come to a decision about having an SCR and whether to include additional information.

Vulnerable patients and carers Having an SCR that includes extra information can be of particular benefit to patients with detailed and complex health problems. If you are a carer for someone and believe that this may benefit them, you could discuss it with them and their GP practice.

NEW PRACTICE BUSINESS MANAGER

We would like to welcome Mrs. Karen Murray as our new Practice Business Manager.

Karen has a great wealth of experience in patient care and business. We are all very much looking forward to working with her and welcoming her into our team.

GP TRAINING PRACTICE

We have recently been approved as a GP Training practice. This means that we will be having fully qualified Doctors coming to the surgery to complete their General Practice Training.

These Doctors will have had considerable experience in hospital medicine, as well as, in many cases, general practice. They will be able to see patients, prescribe medication and make referrals like any other Doctor working in the practice.

Dr Frances Batten will be starting on Monday 30th January and will be with us for approximately 6-12 months

SAVVY Kernow (for 13 - 19 yrs)

We are currently updating our registration with SAVVY Kernow and altering our information: www.savvykernow.org.uk   

The Surgery is also the named ‘Safe Haven for Patients with Learning Disabilities’ within the area.

As part of our serivce to you, we would like to inform you that you have a named allocated GP which is recorded on your medical record. If you would like to know who your named allocated GP is, please ask Reception. This does not mean that you cannot see a GP of your choice as we do not hold individual GP lists and you are registered with the Practice in general.

Request from Devoran Village Hall Committee

The Devoran Village Hall Committee have requested that patients attending Devoran surgery DO NOT park in the Village Hall Car Park AT ANY TIME.

As a concession, patients at the surgery are allowed to use the disabled/loading bay at the front of the village hall by the surgery, if it is not required by a Village Hall user with mobility problems.

The surgery has lined parking spaces in front of the surgery and patients are able to park on the road opposite the surgery.

We appreciate that patients have parked in the Village Hall car park for a number of years, but now that the Village Hall is being hired/used more frequently, the users of the Village Hall have priority at all times as instructed by the Village Hall Committee.

DISPENSARIES


From 1st April 2017 prescription charges in England have increased to £8.60 per item.

EPS (Electronic Prescriptions) has recently been introduced to Cornwall. Unfortunately we currently cannot provide this service but when it becomes available the service will enable prescriptions for Non-Dispensing Patients to be sent electronically for collection to their Pharmacy of choice. This is a service that Dispensing Patients already enjoy from the Practice Dispensary. For further information please click on the header for EPS under Further Information.

ON LINE PRESCRIPTION ORDERING

We have now changed over to 'The Waiting Room 2' for on line prescription ordering.
In order to use 'The Waiting Room 2' you will need to register first by downloading and completing the form below. Please take the completed form to the surgery along with 2 forms of ID (one photographic IE: passport / driving licence) Online Access Application Form.

Please DO NOT attempt to use 'The Waiting Room' unless you have already completed the form and received a confirmation email.

The Waiting Room 2 is a secure system linked to your medical record at your surgery and this allows you to book your appointments, get some confidential online access to your GP records and order repeat prescriptions online. But this is just the start. In the future there will be more services added.

In the meantime you can order via our dedicated automated phone line (01872 562201) or handing in your repeat slip at the Surgery. Please contact the surgery if you have any queries.
