Please could we remind patients that this service is for Non-Urgent medical issues and requests.
We are seeing an increase in patients using this for urgent matters that need advice/support on the same day.

We would advise not using this form for unwell children due to the timescales stated detailed.
All forms submitted will be read by a member of the Admin Team within two working days who will then pass it onto a GP, if appropriate.
If a GP requests a routine appointment with you this will be arranged in line with our current waiting time for routine appointments.
