New Patient Registration

Our Practice area covers the triangle between Redruth, Truro and Penryn. We care for Patients from all the villages within this area.  Unfortunately, we cannot register new Patients outside of our practice boundary. We aim to provide a full range of family care in a  compassionate and professional way.

All Patients who register with the Practice are registered with a named GP who will be responsible for their overall care at the Practice. However, you do have the right to express a preference to receive services from a particular Practitioner.  You may ask to see a Practitioner of your choice at any time, however, this will, of course, be subject to their availability and may mean you could wait longer to see them

Whenever possible, a continuing problem is best dealt with by the same Doctor.

New Patient Forms to Download:

Documents to bring with you when registering:

  • Photo ID (Passport/Driving Licence)
  • Proof of address in catchment area, i.e. Current Utility Bill
  • For children (under 5 years of age) – Red Baby Book
  • For children (Birth – 18 years if no photo ID available) – birth certificate

If Ex Forces:

  • Copy of discharge letter

If Non-British Applicants:

  • Passport containing Visa details

Temporary Patients

If you are visiting from outside our practice area we ask that you contact your own GP first who may be able to sort simple queries, medication issues or decide that they feel you need to be seen.

If you require an urgent medical review, appointments can be made to see a doctor on a temporary basis only after speaking to your home GP.
